The Program Coordinator coordinates the daily operation of the Stew Leonards Employee Medical Services contract location.  This individual monitors patient treatment and assures quality patient care, ensures ongoing Quality Assurance and Performance Improvement, and performs patient care as determined by the scope of their licensure.  The Program Coordinator encourages and engages the Stew Leonard employees to participate in Health and Wellness initiatives, provides supplemental assistance to Stew Leonard employees in regard to managing their health and wellness issues as identified by the employees’ primary care doctor.  Supplemental assistance shall consist of but is not limited to the following:  checking BP, specified diagnostic testing, contacting primary care providers, assisting with monitoring of chronic conditions etc.  The Coordinator tracks employees health and wellness data in designated EMR and maintains contact with Occupational Health Services Medical Directors and contacts Mid-Level Practitioners for support in accordance with departmental guidelines.  This individual utilizes an interdisciplinary team approach to employee Wellness Services between Occupational Health, Stew Leonards and its employees.
Qualified applicant are required to have a current CT and NY RN license and 3-5 years experience in Primary/urgent care, in an outpatient setting preferred.  Current BLS/ACLS certifications are required as well as Phlebotomy, ECG and PPD testing. Successful candidates must be self-directed, with strong interpersonal, decision making, problem solving skills are required.  This individual must have a broad knowledge of general nursing and an appreciation of individual / group behavior, including employee/employer relationships are required.  Compilation, handling, dissemination and processing: of sensitive health data  for reports required.  Must have strong Microsoft office, Internet, Spreadsheet General office equipment skills.  Extensive travel between multiple locations is required.  Flexibility with work schedule dates and time.
